It
is again
time to fill
out Annual
Local Church
Forms.
The
information
gathered on
these forms
is what is
printed in
the National
Yearbook and
is your
church's
official
record of a
Church with
Standing with
the United
Church of
Christ.
We also use
the
information
from these
forms to
figure out
your Per
Capita Dues
and for our
own Vermont
Yearbook
information.
The forms
are posted
on the UCC
website and
available
for download
at:

or at the
links below.
These forms
are designed
to be
downloaded/saved
and filled
out on your
computer.
You will
need Adobe
Reader to
open all
these forms.

Follow the
instructions
located at
this website
for
downloading
and
installing
Adobe Reader
to your
computer).
We ask that
once
downloaded,
filled-out
and saved,
that, if
possible,
you please
email a copy
of the form
as an
attachment
to Lynn
Thomas at
thomasl@vtcucc.org.

followed by
your church
number.

and of
course, save
a copy for
yourself).
Form G -
Local Church
Report
The
preferred
method for
completing
Form G is
via the UCC
online
reporting
system.
You will
receive a
postal
mailing from
the
Conference
Office,
which
includes the
link to the
reporting
website,
your
username and
password, as
well as a
partially
completed
Form G
(containing
your last
year's
numbers).
(NOTE: the
UCC online
reporting
system is
designed to
work with
the Internet
Explorer
browser and
will not
work with
the Foxfire
browser).

for filling
out the
forms and
emailing
them or

February 19,
2010.
Your
churches
information
may not make
it into the
yearbook if
