It is again
the time of
year to fill
out your
Local Church
Forms. The
information
gathered on
these forms
is what is
printed in
the National
Yearbook and
serves as
your
church's
official
record of a
Church with
Standing
with the
United
Church of
Christ. We
also use the
information
from these
forms to
determine
your Per
Capita Dues
and for our
own Yearbook
information.
All the
annual
Yearbook
reporting
forms are
available to
complete
online
(click on
the "Access
Database"
link) or for
download at
www.ucc.org/research/yearbook.html.
We will also
have them
available on
the
Conference
website for
downloading
at
www.vtcucc.org/churchforms.html.
Please
remember to
keep a copy
of the
filled out
forms for
yourself
either
printed or
electronically.
Simply
download a
copy of each
form and you
can complete
it on your
computer.
You will
need to have
Adobe
Acrobat
Reader in
order to do
this. If you
do not have
this program
it can be
downloaded
for free at
http://get.adobe.com/reader/.
Follow the
instructions
provided on
this website
to download
and install.
We would
like as many
people as
possible to
complete the
forms online
using the
online
system.
Forms can
also be
emailed to
Lynn Thomas

or mailed to
the
Conference
office.
(Remember to
save the
blank forms
to your
computer and
then save
them again
after
entering
information
before
trying to
email them
to the
Office.)

When
downloading
the forms
above, save
them as
files on
your
computer.
Each time
you enter
data/information,
save them
again.
Save a final
time before
emailing to
the
Conference
Office.
(If you
don't save
the forms to
your
computer,
and after
entering
information,
your data
will not be
saved - the
form will be
blank.)
